I can't understand why the US figure is so low.  Presumably the
anti-vaxxer brigade (Jones et al ) is much stronger than
elsewhere?  

Less testing is probably your answer. Politicians are arguing for that approach here also. And to revert from free tests to paid ones. Both will lower the numbers you see in the daily reports, but it won't lower the actual infection rates.
