When comparing x/per million pop you do this exactly to factor out the total population. Hence your remark of "with four times the US population" makes no sense in this context.

While I don't know, and certainly can't speak for SYZ, I think he may be pointing up the greater population as a way of highlighting India's vastly hight population density.

Yes, the high population density combined with the low rates makes me wonder indeed. but hey, mybe thats the small unconscious racist in me (and all of us) who cant fathom brown(ish) people to be able to be at least as smart and disciplined as we are.
